JOB DESCRIPTION SUMMARY
The Care Team Case Manager provides direct support to students who are experiencing challenges that impact their academic and social success, such as physical or mental health, social or academic adjustment issues. Specific responsibilities will involve regular student meetings and triaging of issues, and direct case management that supports student well-being and success.

JOB DESCRIPTION

BASIC FUNCTIONS

The Care Team Case Manager provides direct support to students who are experiencing challenges that impact their academic and social success, such as physical or mental health, social or academic adjustment issues. Specific responsibilities will involve regular student meetings and triaging of issues, and direct case management that supports student well-being and success.

PRIMARY RESPONSIBILITIES
  • Facilitate the weekly CARE Team meeting to report upon, monitor and address updates on individual student situations.
  • Monitor, triage, and respond to "I'm Concerned about a Student" submissions and other forms of outreach regarding students of concern.
  • Manage and update the Symplicity CARE System database.
  • Connect with students (and their parents/guardians as appropriate) via face-to-face, phone or Zoom sessions to develop individual action plans, and track progress towards personal and academic goal achievement.
  • Follow up and track cases to ensure university resources have been made available and/or mobilized to students.
  • Formulate intervention plans that support and enhance student academic and social success, including working with the Emergency Assistance Fund, Student Success Fund, Food Insecurity Fund, or other avenues of assistance for students.
  • Meet with students and their parents/guardians to discuss leave of absences and/or withdrawals. Serve on the medical withdrawal committee, and work in coordination with other university departments to track the student re-entry process to WPI.
  • Serve as a key integrator across the campus to assure that current and incoming students are aware of and leverage all available support services.
  • Develop and distribute awareness campaigns and training programs to increase awareness of the CARE Team, reporting and supporting students in distress, and campus resources.
  • Work in partnership with the Office of Accessibility Services to assist students who may need temporary accommodations.
  • Serve as the primary point of contact for the preferred name process. Meet with students to educate them about the process and work in partnership with campus offices to initiate and facilitate preferred name changes in various university systems.
  • Participate fully as a member of the Dean of Students staff in campus events and programming.
  • Serve as an Insight Advisor for incoming students, as needed.
  • Assist as needed in the Dean of Students Office, including with Student Conduct related matters.
  • Serve within the Dean of Students Administrator-on-Call rotation cycle.
  • Other duties as assigned.


REQUIREMENTS
  • A master's degree in an appropriate field, such as Education, Counseling, Social Work, Public Health or a related field.
  • Two (2) years of experience working as part of a case management support team, with preference towards those candidates with direct experience with college aged populations.
  • Ability to handle personal and confidential information with discretion.
  • Experience using case management software, such as Symplicity or Maxient preferred.
  • A sincere and demonstrated passion for college student success.
